Our Programs
We specialize in three core Medicaid waiver programs that allow eligible Monroe County residents to receive care at home rather than in a nursing facility.
The NHTD Waiver is designed for Monroe County adults who are at risk of nursing home placement or who are currently living in a facility and want to return home. Priority Cares coordinates the full range of NHTD services — including home modifications, assistive technology, and personal care — so participants can live on their own terms. Our NHTD care coordinators are based in the Rochester area and understand the local resources available through Monroe County's Department of Human Services. Eligibility is open to adults 18 and older who meet Medicaid and functional criteria.
Medicaid CoveredTraumatic brain injury can profoundly change daily life, and recovery often requires ongoing, personalized support. The TBI Waiver provides funding for a broad set of services including community integration counseling, independent living skills training, and behavioral supports — all delivered in the home or community setting. Priority Cares pairs each TBI participant with a dedicated support coordinator who monitors progress and adjusts the plan of care as needs evolve. Our team has direct experience navigating Rochester-area TBI referral networks, including Strong Memorial Hospital and Unity Health.
Medicaid CoveredThe Personal Care Aide program funds hands-on daily assistance for Medicaid-eligible Monroe County residents who need help with bathing, dressing, grooming, meal preparation, and light housekeeping. Priority Cares employs trained, background-checked PCAs who are matched to each client based on language, schedule, and care needs. All of our aides receive ongoing supervision from a registered nurse to ensure consistent quality of care. Authorizations are obtained directly through the Monroe County Local Department of Social Services, and our team handles all the paperwork on your behalf.

Getting Started
Starting Medicaid home care can feel overwhelming. We make it straightforward.
Call us at (718) 400-6166 or complete our online intake form. A care coordinator will review your Medicaid status, diagnosis, and functional needs to determine which program — NHTD, TBI, or PCA — is the right fit. This call is completely free and takes about 15 minutes.
Once eligibility is confirmed, our team submits all documentation to Monroe County LDSS and/or the NHTD/TBI Regional Resource Development Center on your behalf. We track every step of the authorization process and keep you updated in plain language — no jargon, no guesswork.
After authorization is received, we match you with a qualified aide or support worker and build a schedule that fits your life. A registered nurse conducts an in-home assessment and supervises ongoing care to ensure quality and safety. Most clients begin services within 7–14 days of authorization.
