Priority Cares provides NHTD and TBI waiver services in all five New York City boroughs: service coordination, independent living skills training, community integration counseling and positive behavioral supports, so you can live independently in your own home.
New York State offers two Medicaid waivers for people who want to live at home instead of a nursing facility. Priority Cares is approved for both in the New York City region, for the four services listed below.
Nursing Home Transition & Diversion — For individuals with physical disabilities who want to live in the community instead of a nursing facility.
Traumatic Brain Injury — For individuals 18–64 who have survived a TBI and need support to remain in the community.
Where we are approved: Priority Cares is approved by the NYS Department of Health for the NHTD waiver (provider 07186336) and the TBI waiver (provider 07529468) in the New York City region, all five boroughs, for Service Coordination, Independent Living Skills Training, Community Integration Counseling and Positive Behavioral Interventions & Supports. On Long Island (Nassau and Suffolk) we are approved for NHTD Community Integration Counseling only. The waivers also fund other services, such as home care aides, respite, home modifications and assistive technology, through other approved providers; your Service Coordinator arranges those. If you live outside our approved area, we can help you find an approved provider through the Regional Resource Development Center (RRDC) for your county.
A note on NHTD right now: New York State is currently not accepting most new referrals to the NHTD waiver, and a limited number of openings are released through the Regional Resource Development Centers (RRDCs). If you are already on NHTD, you choose your own providers and can ask to change them. If you are not enrolled yet, we will explain the process and help you stay in touch with your RRDC. How the process works: Referrals for NHTD and TBI waiver services are coordinated through the Regional Resource Development Centers (RRDCs). Our care coordinators work directly with RRDCs to streamline your enrollment and get services started quickly. If you've already been evaluated, we can often begin within days of approval.
You may be eligible for NHTD or TBI waiver services if you meet the following criteria:
You must be enrolled in NY Medicaid. We can help you apply if you're not yet enrolled.
Acquired brain injury from trauma, stroke, or other causes. Must need home-based support to remain in the community.
18+ with physical disability risk of nursing home placement who want to remain in or return to community living.
You want to live in your own home, apartment, or a community setting — not a nursing facility.
An RRDC assessment will determine the level of services you need. We accompany you through this process.
We provide NHTD and TBI waiver services in all 5 boroughs. On Long Island we provide NHTD Community Integration Counseling. Elsewhere, we help you find an approved provider through your RRDC.

We refer you to the appropriate Regional Resource Development Center. Our team prepares your documents and attends the assessment with you (or advocates on your behalf).
Once approved, your Service Coordinator and Priority Cares develop a personalized care plan — with Priority Cares providing the services we are approved for and your Service Coordinator arranging any others through approved providers.
Services start. You receive regular check-ins from your Service Coordinator, and your plan adjusts as your needs change.
