Priority Cares is an approved Traumatic Brain Injury (TBI) Waiver provider in the New York City region. We serve adults 18–64 who have survived a brain injury and need support to remain in the community, in all five boroughs.

Where we are approved: Priority Cares is approved by the NYS Department of Health for the NHTD waiver (provider 07186336) and the TBI waiver (provider 07529468) in the New York City region, all five boroughs, for Service Coordination, Independent Living Skills Training, Community Integration Counseling and Positive Behavioral Interventions & Supports. On Long Island (Nassau and Suffolk) we are approved for NHTD Community Integration Counseling only. The waivers also fund other services, such as home care aides, respite, home modifications and assistive technology, through other approved providers; your Service Coordinator arranges those. If you live outside our approved area, we can help you find an approved provider through the Regional Resource Development Center (RRDC) for your county.
A note on NHTD right now: New York State is currently not accepting most new referrals to the NHTD waiver, and a limited number of openings are released through the Regional Resource Development Centers (RRDCs). If you are already on NHTD, you choose your own providers and can ask to change them. If you are not enrolled yet, we will explain the process and help you stay in touch with your RRDC. What is the TBI Waiver? The NY TBI Waiver (Traumatic Brain Injury Medicaid Waiver) is a federal-state Medicaid program specifically for adults 18–64 who have acquired a brain injury from trauma, stroke, or other causes. It funds specialized services not available under standard Medicaid — helping survivors stay in their community instead of a nursing facility.
Eligibility criteria for the New York TBI Waiver:
Brain injury from trauma, stroke, anoxia, tumor, infection, or other non-progressive neurological cause.
The TBI Waiver serves adults between 18 and 64 years of age at time of enrollment.
Must be enrolled in NY Medicaid. We help if you're not yet enrolled.
You want to live at home — not in a nursing facility or rehabilitation center.
Your TBI-related care needs meet the nursing level of care standard (RRDC-assessed).
We provide TBI waiver services in the five NYC boroughs. Outside NYC, we help you find an approved provider through your RRDC.

We submit your referral to the Regional Resource Development Center and prepare medical documentation, Medicaid verification, and all required paperwork.
An RRDC assessor evaluates your TBI-related care needs. Our coordinator attends the assessment to advocate for the full scope of services you need.
Once approved, your individualized service plan is developed. Priority Cares provides the services we are approved for, and your Service Coordinator arranges any others through approved providers.
