Monroe County home care › For families

For families

Arranging care for a parent? We'll keep you in the loop.

Most of our calls come from a son, daughter or spouse. You do not have to have every answer before you call.

Before you call

Helpful to have ready (but not required)

  • Your relative's town and the best number to reach them and you
  • Whether they have Medicaid now, and whether it goes through Monroe County or a health plan
  • Whether they have had the New York Independent Assessor assessment, or a letter from the county
  • What a normal day looks like and where they need help: mornings, bathing, meals, getting around
  • Any upcoming hospital discharge date

Please do not send Medicaid ID or medical record numbers through our web form. We will ask for what we need on the phone.

Speaking for someone else

Your relative chooses their agency, so we will want to speak with them too, or with the person who has legal authority to make decisions for them. If they agree, we can keep you involved in scheduling and updates.

Leaving the hospital?

Tell the discharge planner at Strong, Highland, Rochester General or Unity that you would like Priority Cares Home Services, and call us the same day so we can be ready.

Caring for yourself, too. Monroe County Office for the Aging has caregiver support and information: (585) 753-6280, 435 East Henrietta Road, Rochester.

Ask us to call you

Not sure where you are in the process?

That is okay. Call our Rochester office, Monday to Friday, 9:00 AM to 5:00 PM, and we will walk you through it.
