Medicaid home care arranged through Monroe County
If you have Medicaid and your home care goes through Monroe County directly (not through a managed care plan), the Monroe County Department of Human Services oversees your personal care. The county gives people approved for personal care a list of agencies to choose from, and Priority Cares Home Services is on that list.
In a Medicaid managed care plan instead? Call us anyway. We will tell you honestly whether we can help and who to call if we cannot.
What New York looks for
To get personal care, adults on Medicaid have an assessment by the New York Independent Assessor. It looks at how much help you need with everyday activities. Since September 2025, adults 21 and over who are new to personal care generally need to:
- need at least limited hands-on help with more than two everyday activities such as bathing, dressing, toileting, eating or moving around, or
- if they have a diagnosis of dementia or Alzheimer's, need at least supervision with more than one everyday activity.
The assessment decides eligibility and how many hours of help are approved. We cannot decide that, but we can help you get ready for it.
Age 65 or older, or have a disability: call Monroe County at (585) 753-6960 for an application (the office is at 111 Westfall Road, Rochester).
