How to Choose the Right Home Care Agency in NYC: A Family’s Complete Guide

Choosing a home care agency for a family member is one of the most important decisions you’ll make. In New York City, hundreds of licensed home care agencies compete for your business — but not all of them offer the same level of care, compliance, or support.
This guide walks you through what to look for and what questions to ask.
Step 1: Verify the Agency Is LHCSA-Licensed
In New York State, any agency providing home health aide services must be licensed by the Department of Health as a Licensed Home Care Services Agency (LHCSA). This is not optional — it’s the law.
Ask any agency: “Can I see your LHCSA certificate?”
A legitimate agency will provide it without hesitation.
Step 2: Understand How They Source and Train Caregivers
Some agencies use a staffing pool — pulling from a registry of independent workers without direct oversight. Others employ their aides directly, with background checks, ongoing training, and supervisory support.
What to ask:
- Are your aides employees or independent contractors?
- What background screening do you conduct?
- How much training do aides receive beyond the state minimum?
- Who supervises the aide after placement?
Direct employment creates accountability. If an aide calls out sick, an employer-agency can send a replacement. A registry cannot.
Step 3: Verify They Accept Your Insurance or Waiver Program
Not all agencies accept Medicaid, NHTD waiver, TBI waiver, or MLTC insurance. Many serve private pay clients only.
Programs and payment types to ask about:
- NHTD Waiver (Nursing Home Transition & Diversion)
- TBI Waiver (Traumatic Brain Injury)
- MLTC (Managed Long-Term Care plans)
- Private pay and long-term care insurance
Before you spend hours onboarding with an agency, confirm they accept your specific insurance or waiver.
Step 4: Ask About Response Times
When your regular aide is unavailable, how long does the agency take to provide coverage? Ask for a specific answer, including how call-outs are handled on weekends.
Step 5: Understand the Service Area
Many NYC agencies are licensed citywide but only actively recruit in one or two boroughs. Ask any agency where it actually has aides working, and be specific about your neighborhood.
