Our Services
Priority Cares Home Services (PCHS) helps New Yorkers live safely and independently at home. Our care team builds a personalized plan across three core programs — NHTD Waiver, TBI Waiver, and PCA Services — so you or your loved one gets exactly the right level of support. NHTD and TBI waiver services are provided in New York City (on Long Island, NHTD Community Integration Counseling only); PCA services are provided in Chemung, Genesee, Livingston, Monroe and Ontario counties.
NHTD Waiver
The Nursing Home Transition and Diversion (NHTD) waiver helps Medicaid-eligible seniors and adults with physical disabilities receive service coordination, independent living skills training, and community integration counseling — so they can live safely in the community instead of a nursing facility.
TBI Waiver
The Traumatic Brain Injury (TBI) Medicaid Waiver is a Home and Community-Based Services (HCBS) program administered by the New York State Department of Health. It gives individuals recovering from brain injury access to service coordination, skills training, and counseling built around their recovery goals.
PCA Services
For people in Chemung, Genesee, Livingston, Monroe and Ontario counties who need companionship and support with everyday safety and wellbeing, our Patient Care Assistants provide compassionate in-home care — help with daily activities, medication reminders, and meaningful companionship that keeps clients connected and comfortable at home. In New York City, Westchester and Long Island, we’ll connect you with a licensed home care agency and help you through the Medicaid and MLTC steps.
