If a nursing home is not your preferred path — or you wish to transition out — NHTD empowers you to live independently with community-based support. If you are already on the NHTD waiver in New York City, you can choose or switch to Priority Cares for service coordination, which helps arrange services and equipment through approved providers so you can thrive safely at home. PCA (Personal Care Assistance)
Everyone needs a helping hand sometimes. In our licensed upstate counties (Chemung, Genesee, Livingston, Monroe and Ontario), our trained PCA caregivers assist with everyday tasks like bathing, dressing, mobility, and meal prep — allowing clients to maintain their routines with dignity. In New York City we don’t send aides, but we’ll connect you with a licensed home care agency. Priority Cares Home Services
TBI (Traumatic Brain Injury) Support
In the aftermath of a brain injury, the path to recovery is full of challenges. In New York City, our TBI waiver services (service coordination, independent living skills training, community integration counseling and positive behavioral supports) help clients regain skills and reintegrate into daily life.
