By leveraging Medicaid-funded programs, namely Nursing Home Transition & Diversion (NHTD) and Traumatic Brain Injury (TBI) waiver services in New York City and Personal Care Assistance (PCA) in our licensed upstate counties around Rochester, we provide the tools and resources necessary for individuals to thrive in the comfort of their own homes.

Traumatic Brain Injury (TBI) Support

Recovering from a brain injury is a journey that requires patience, understanding, and specialized care. In New York City, our TBI waiver services (service coordination, independent living skills training, community integration counseling and positive behavioral supports) help individuals regain independence and live safely at home.

Nursing Home Transition & Diversion (NHTD)

Transitioning from institutional care to community living can be daunting. Through the NHTD program, we support seniors and individuals with disabilities in New York City who are already on the waiver in moving from or avoiding nursing home placement, with service coordination, independent living skills training, community integration counseling and behavioral supports, while other waiver services come from approved providers. Current NHTD participants can choose or switch providers; the state is currently not accepting most new NHTD referrals, so for new clients we can check TBI waiver eligibility.

Personal Care Assistance (PCA)

Maintaining daily routines is essential for independence and well-being. In our licensed upstate counties (Chemung, Genesee, Livingston, Monroe and Ontario), our PCA program offers trained caregivers who assist with activities of daily living such as bathing, dressing, meal preparation, and medication reminders. By providing compassionate support, we help individuals maintain their dignity and quality of life in a familiar environment. We don’t send home care aides in New York City, but we’ll connect you with a licensed home care agency and walk you through Medicaid and MLTC.
