What We Do
At Priority Cares, we believe that every individual deserves the opportunity to live with dignity and independence. Our services are designed to provide the necessary support to achieve this goal:
- Traumatic Brain Injury (TBI) Support: In New York City, we assist individuals in regaining independence after a brain injury, offering coordinated support services to help them live safely at home instead of in a nursing facility.
- Nursing Home Transition & Diversion (NHTD): This Medicaid program supports seniors and individuals with disabilities in transitioning from or avoiding nursing home care by offering community-based support, allowing them to live life on their terms. In New York City, people already on the NHTD waiver can choose Priority Cares for service coordination, independent living skills training, community integration counseling and behavioral supports.
- Personal Care Assistance (PCA) Program: In our licensed upstate counties (Chemung, Genesee, Livingston, Monroe and Ontario), individuals receive help through PCA with daily tasks like bathing, dressing, and meals, provided by trained caregivers in the comfort of their own home.

In New York City we are a NYS DOH-approved NHTD and TBI waiver provider (service coordination, independent living skills training, community integration counseling and behavioral supports). We don’t send home care aides in New York City, but we’ll connect you with a licensed home care agency and walk you through Medicaid and MLTC. If you or your loved one is already on the NHTD waiver, you can choose or switch your provider; for new clients, we can check TBI waiver eligibility. Personal care aides are available from Priority Cares only in our licensed upstate counties (Chemung, Genesee, Livingston, Monroe and Ontario).
